To be eligible for our services, you must:
- Have spent at least 1 day in foster care
- Be at least 18 years of age or entering college
- Be currently living in Tennessee
This application serves as an opportunity to verify your eligibility for our services, help us learn more about you (your current situation and future goals), and how we may best support you.
Next Steps
After you submit your application, the Youth Services Connection Coordinator will contact/email you within two business days with more information on the following steps:
- Submit verification documents that you were in foster care (dependency letter). If you do not have or cannot access the document, we will provide more information on how to get one.
- Scheduling your Omni Family Foundation welcome meeting
